You are eligible for a new passport if you are age 16 or over and ONE of the following is true:

• You are applying for your first U.S. passport.
• Your previous U.S. passport was issued when you were under age 16.
• Your previous U.S. passport was issued more than 15 years ago.

You are eligible for a passport renewal if ALL of the following are true:

• Your passport is currently valid, or has expired within the last five years.
• Your passport is in your possession.
• Your passport is not damaged.
• Your passport was issued when you were age 16 or older.

You may request a passport renewal for any one of the following reasons:

• Your passport is expired or will expire soon.
• Your passport is low on blank visa pages.
• You have legally changed your name, and your passport was issued more than one year ago.

Select this service if the child is under 16 years of age, even if the child has previously been issued a passport.

Please note:

• BOTH parents must appear with child at a local acceptance agent.
• If one of the parents or legal guardians cannot be present at the acceptance agent, then Form DS-3053 (Minor Consent Form) must be notarized.

Select this service if your most recent passport was lost or stolen, even if your passport was expired.

Please note:

• You must complete both the DS-11 and DS-64 (Statement Regarding Lost or Stolen Passport) forms.
• A police report is not required.

You should choose this service if your most recent passport was severely damaged. Examples of severe damage include:

• Unauthorized markings on the information/photo page
• Loose or missing cover
• Cuts or tears on any page or the cover

Select this service if your most recent passport was lost or stolen, even if your passport was expired.

Please note:

• You must complete both the DS-11 and DS-64 (Statement Regarding Lost or Stolen Passport) forms.
• A police report is not required.

You are eligible for passport name change service if:

• Your current passport is valid, in your possession, and undamaged.
• You have legally changed your name since your passport was issued.
• You are able to submit your original proof of name change (i.e. marriage certificate, divorce decree, or court order).

If you meet one of the following criteria you may request a second passport:

• If you are a frequent traveler and need to obtain a visa stamp in your current valid 10 year passport while you are traveling abroad.
• If you are traveling between politically sensitive countries.
